Participation Agreement *

By submitting this form, I affirm that I have represented myself and my work truthfully and accurately. The Arts Council of Park City & Summit County reserves the right to decline entries deemed inappropriate for this event. I allow photos that might be taken of my work for Arts Council and event publicity purposes. If selected, I understand that all registrations and payment must be received no later than February 5, 2021. Registration fees for the 2021 Winter Arts Showcase are $60. The cancellation deadline is February 5th and refunds will not be given after that date. If selected, I agree to select no more than 10 items for the Winter Arts Showcase online at createpc.org, agree to NOT select items that are for sale on other online platforms, understand that I am responsible shipping and delivering my product to a customer if it is sold. If for any reason you are unable to participate in the event, as a courtesy to other artists, please notify us immediately at 435-647-9747.
